Project description:Serial endoscopic examinations of a patient are important for early diagnosis of malignancies in the gastrointestinal tract. However, retargeting for optical biopsy is challenging due to extensive tissue variations between examinations, requiring the method to be tolerant to these changes whilst enabling real-time retargeting.This work presents an image retrieval framework for inter-examination retargeting. We propose both a novel image descriptor tolerant of long-term tissue changes and a novel descriptor matching method in real time. The descriptor is based on histograms generated from regional intensity comparisons over multiple scales, offering stability over long-term appearance changes at the higher levels, whilst remaining discriminative at the lower levels. The matching method then learns a hashing function using random forests, to compress the string and allow for fast image comparison by a simple Hamming distance metric.A dataset that contains 13 in vivo gastrointestinal videos was collected from six patients, representing serial examinations of each patient, which includes videos captured with significant time intervals. Precision-recall for retargeting shows that our new descriptor outperforms a number of alternative descriptors, whilst our hashing method outperforms a number of alternative hashing approaches.We have proposed a novel framework for optical biopsy in serial endoscopic examinations. A new descriptor, combined with a novel hashing method, achieves state-of-the-art retargeting, with validation on in vivo videos from six patients. Real-time performance also allows for practical integration without disturbing the existing clinical workflow.

Project description:The inability to identify microscopic tumors and assess surgical margins in real-time during oncologic surgery leads to incomplete tumor removal, increases the chances of tumor recurrence, and necessitates costly repeat surgery. To overcome these challenges, we have developed a wearable goggle augmented imaging and navigation system (GAINS) that can provide accurate intraoperative visualization of tumors and sentinel lymph nodes in real-time without disrupting normal surgical workflow. GAINS projects both near-infrared fluorescence from tumors and the natural color images of tissue onto a head-mounted display without latency. Aided by tumor-targeted contrast agents, the system detected tumors in subcutaneous and metastatic mouse models with high accuracy (sensitivity = 100%, specificity = 98% ± 5% standard deviation). Human pilot studies in breast cancer and melanoma patients using a near-infrared dye show that the GAINS detected sentinel lymph nodes with 100% sensitivity. Clinical use of the GAINS to guide tumor resection and sentinel lymph node mapping promises to improve surgical outcomes, reduce rates of repeat surgery, and improve the accuracy of cancer staging.

Project description:In the past decades, image-guided surgery has evolved rapidly. In procedures with a relatively fixed target area, like neurosurgery and orthopedics, this has led to improved patient outcomes. In cancer surgery, intraoperative guidance could be of great benefit to secure radical resection margins since residual disease is associated with local recurrence and poor survival. However, most tumor lesions are mobile with a constantly changing position. Here, we present an innovative technique for real-time tumor tracking in cancer surgery. In this study, we evaluated the feasibility of real-time tumor tracking during rectal cancer surgery. The application of real-time tumor tracking using an intraoperative navigation system is feasible and safe with a high median target registration accuracy of 3 mm. This technique allows oncological surgeons to obtain real-time accurate information on tumor location, as well as critical anatomical information. This study demonstrates that real-time tumor tracking is feasible and could potentially decrease positive resection margins and improve patient outcome.

Project description:BackgroundElectromagnetic tracking is a relatively new technique that allows real time navigation in the absence of radiation. The aim of this study was to prove the feasibility of this technique for the treatment of posterior pelvic ring fractures and to compare the results with established image guided procedures.MethodsTests were performed in pelvic specimens (Sawbones®) with standardized sacral fractures (Type Denis I or II). A gel matrix simulated the operative approach and a cover was used to disable visual control. The electromagnetic setup was performed by using a custom made carbon reference plate and a prototype stainless steel K-wire with an integrated sensor coil. Four different test series were performed: Group OCT: Optical navigation using preoperative CT-scans; group O3D: Optical navigation using intraoperative 3-D-fluoroscopy; group Fluoro: Conventional 2-D-fluoroscopy; group EMT: Electromagnetic navigation combined with a preoperative Dyna-CT. Accuracy of screw placement was analyzed by standardized postoperative CT-scan for each specimen. Operation time and intraoperative radiation exposure for the surgeon was documented. All data was analyzed using SPSS (Version 20, 76 Chicago, IL, USA). Statistical significance was defined as p< 0.05.Results160 iliosacral screws were placed (40 per group). EMT resulted in a significantly higher incidence of optimal screw placement (EMT: 36/40) compared to the groups Fluoro (30/40; p< 0.05) and OCT (31/40; p< 0.05). Results between EMT and O3D were comparable (O3D: 37/40; n.s.). Also, the operation time was comparable between groups EMT and O3D (EMT 7.62 min vs. O3D 7.98 min; n.s.), while the surgical time was significantly shorter compared to the Fluoro group (10.69 min; p< 0.001) and the OCT group (13.3 min; p< 0.001).ConclusionElectromagnetic guided iliosacral screw placement is a feasible procedure. In our experimental setup, this method was associated with improved accuracy of screw placement and shorter operation time when compared with the conventional fluoroscopy guided technique and compared to the optical navigation using preoperative CT-scans. Further studies are necessary to rule out drawbacks of this technique regarding ferromagnetic objects.

Project description:Brain-computer interfaces (BCIs) allow people with severe neurological impairment and without ability to control their muscles to regain some control over their environment. The BCI user performs a mental task to regulate brain activity, which is measured and translated into commands controlling some external device. We here show that healthy participants are capable of navigating a robot by covertly shifting their visuospatial attention. Covert Visuospatial Attention (COVISA) constitutes a very intuitive brain function for spatial navigation and does not depend on presented stimuli or on eye movements. Our robot is equipped with motors and a camera that sends visual feedback to the user who can navigate it from a remote location. We used an ultrahigh field MRI scanner (7 Tesla) to obtain fMRI signals that were decoded in real time using a support vector machine. Four healthy subjects with virtually no training succeeded in navigating the robot to at least three of four target locations. Our results thus show that with COVISA BCI, realtime robot navigation can be achieved. Since the magnitude of the fMRI signal has been shown to correlate well with the magnitude of spectral power changes in the gamma frequency band in signals measured by intracranial electrodes, the COVISA concept may in future translate to intracranial application in severely paralyzed people.

Project description:Intravital microscopy is a powerful technique to observe dynamic processes with single-cell resolution in live animals. No intravital window has been developed for imaging the colon due to its anatomic location and motility, although the colon is a key organ where the majority of microbiota reside and common diseases such as inflammatory bowel disease, functional gastrointestinal disorders, and colon cancer occur. Here we describe an intravital murine colonic window with a stabilizing ferromagnetic scaffold for chronic imaging, minimizing motion artifacts while maximizing long-term survival by preventing colonic obstruction. Using this setup, we image fluorescently-labeled stem cells, bacteria, and immune cells in live animal colons. Furthermore, we image nerve activity via calcium imaging in real time to demonstrate that electrical sacral nerve stimulation can activate colonic enteric neurons. The simple implantable apparatus enables visualization of live processes in the colon, which will open the window to a broad range of studies.

Project description:Registration of three-dimensional ultrasound (3DUS) volumes is necessary in several applications, such as when stitching volumes to expand the field of view or when stabilizing a temporal sequence of volumes to cancel out motion of the probe or anatomy. Current systems that register 3DUS volumes either use external tracking systems (electromagnetic or optical), which add expense and impose limitations on acquisitions, or are image-based methods that operate offline and are incapable of providing immediate feedback to clinicians. This paper presents a real-time image-based algorithm for rigid registration of 3DUS volumes designed for acquisitions in which small probe displacements occur between frames. Described is a method for feature detection and descriptor formation that takes into account the characteristics of 3DUS imaging. Volumes are registered by determining a correspondence between these features. A global set of features is maintained and integrated into the registration, which limits the accumulation of registration error. The system operates in real-time (i.e. volumes are registered as fast or faster than they are acquired) by using an accelerated framework on a graphics processing unit. The algorithm's parameter selection and performance is analyzed and validated in studies which use both water tank and clinical images. The resulting registration accuracy is comparable to similar feature-based registration methods, but in contrast to these methods, can register 3DUS volumes in real-time.

Project description:Positioning accuracy is a challenging issue for location-based applications using a low-cost global positioning system (GPS). This paper presents an effective approach to improving the positioning accuracy of a low-cost GPS receiver for real-time navigation. The proposed method precisely estimates position by combining vehicle movement direction, velocity averaging, and distance between waypoints using coordinate data (latitude, longitude, time, and velocity) of the GPS receiver. The previously estimated precious reference point, coordinate translation, and invalid data check also improve accuracy. In order to evaluate the performance of the proposed method, we conducted an experiment using a GARMIN GPS 19xHVS receiver attached to a car and used Google Maps to plot the processed data. The proposed method achieved improvement of 4-10 meters in several experiments. In addition, we compared the proposed approach with two other state-of-the-art methods: recursive averaging and ARMA interpolation. The experimental results show that the proposed approach outperforms other state-of-the-art methods in terms of positioning accuracy.
