Safety and Efficacy of Atuzabrutinib Topical Gel in Patients with Mild-to-Moderate Atopic Dermatitis: Findings from Phase 2a Trial

ABSTRACT: This phase 2a study of atuzabrutinib (PRN473/SAR444727), a Bruton tyrosine kinase inhibitor, comprised of a double-blind placebo-controlled treatment period and an open-label uncontrolled treatment period and evaluated safety and efficacy of atuzabrutinib 5% gel vs placebo (in double-blind period) in patients with mild-to-moderate atopic. dermatitis (AD). Patients aged 18–70 years diagnosed with mild-to-moderate AD (N=39) for at least 6 months were included in the study. During the double-blinded Period (Days 1–14), two target lesions with a difference ≤1 point in total sign score (TSS) were randomized 1:1 to atuzabrutinib or matching placebo twice daily. During open-label period (Days 15–42), patients continued to treat the assigned areas and other AD-affected lesions (except scalp, palms, soles and genitals) with atuzabrutinib BID. The objective is to evaluate the safety, tolerability, PK and preliminary efficacy of atuzabrutinib in patients with mild to moderate AD. RNA sequencing analysis was performed to understand the target immune cell expression in lesional skin biopsies versus normal skin and the effect of atuzabrutinib.

Fezakinumab (ILV-094; anti IL-22 monoclonal antibody) monotherapy was administered for 12 weeks (primary endpoint), and clinical responses were followed until week 20. AD transcriptome significantly improved at week 12 in fezakinumab vs. placebo (p<1E-18).

Ropinirole hydrochloride identified by iPSC drug discovery for amyotrophic lateral sclerosis: a single-centered, randomized, double-blind, placebo-controlled phase 1/2a trial

Project description:We assessed the safety and efficacy of ropinirole, a candidate drug for amyotrophic lateral sclerosis (ALS) identified using induced pluripotent stem cells (iPSCs) technology, by a randomized controlled trial (UMIN000034954). Twenty participants were randomly assigned to ropinirole or placebo for 24 weeks in the double-blind period, followed by a 24-week open-label active extension period. Primary outcomes were safety and tolerability. Secondary outcomes evaluated the therapeutic effects of ropinirole. Adverse events were mostly those previously reported, with similar rates in both groups. Participants in the ropinirole group had lived an additional 27.9 weeks without disease progression compared with the placebo group at 12 months. We validated and confirmed the efficacy of ropinirole in patients with ALS using the largest publicly available real-world registry database of ALS (PRO-ACT). The iPSCs-derived motor neurons from the participants showed the D2R expression and the potential involvement of the SREBP2-cholesterol synthetic pathway in the therapeutic effects.

Down-regulation of Interferon signature in systemic lupus erythematosus patients by active immunization with Interferon alpha-Kinoid

Project description:We performed a phase I/II, randomized, double-blind, placebo-controlled dose-escalation study to examine the safety, immunogenicity, and biological effects of active immunization with interferon alpha-Kinoid (IFN-K) in systemic lupus erythematosus (SLE) patients. Women 18-50 years of age with mild to moderate SLE were immunized with three (n=10) or four doses (n=9) of 30, 60, 120, 240 microgram IFN-K or saline. Anti-IFNalpha antibodies were detected in all IFN-K-immunized patients. Transcriptomic analysis separated patients at baseline into type I IFN signature-positive and -negative groups. IFN-K induced higher anti-IFNalpha titers in signature-positive than in signature-negative patients and, in signature-positive patients, reduced the expression of IFN-induced genes. The decrease in IFN score correlated with the anti-IFNalpha antibody titers, and with baseline IFN score. IFN-K or placebo was administered at day 0, day 7, day 28 in all patients. Efficacy and safety of ustekinumab treatment in adults with moderate-to-severe atopic dermatitis

Project description:This was a phase II, randomized, placebo-controlled, double-blinded single center study (clinicaltrials.gov: NCT01806662) to investigate safety and efficacy of ustekinumab treatment in moderate-to-severe AD patients. Patients underwent 1:1 randomization using a computer generated subject randomization table by an unblinded pharmacist. to Subjects received subcutaneous ustekinumab or placebo at weeks 0, 4, and 16 with a crossover to the other agent (either ustekinumab or placebo) at weeks 16, 20, and 32 (Figure 1A) to ensure patient retention.
