Project description:In this placebo-controlled randomized controlled trial, we tested whether remote ischemic preconditioning (RIPC) elicited by four 5-minute cycles of 300 mmHg of cuff inflation/deflation of the lower limb would reduce myocardial necrosis in isoflurane-anesthetized patients undergoing on-pump coronary artery bypass graft surgery. Secondary outcomes were the perioperative release of the biomarkers NTproBNP, hsCRP, S100, atrial transcriptional profiles, and short- and long-term clinical outcomes. RIPC with concomitantly applied isoflurane did not affect the release of biomarkers or clinical outcome. NTproBNP release correlated with isoflurane- but not RIPC-induced transcriptional changes.

The primary research question is whether gene expression differs in patients treated with Cogntive Behavioral Stress Management vs control conditions. Project description:To evaluate whether clarithromycin improves 28-day mortality among patients with sepsis, respiratory and multiple-organ dysfunction syndrome. The INntravenous CLArithromycin in Sepsis and multiple organ dysfunction Syndrome trial was a phase 3, randomized, double blind, placebo-controlled clinical study, conducted in 11 intensive care units and 2 Internal Medicine wards in 2 countries. Patients with sepsis, respiratory failure and total sequential organ failure assessment score of ≥7 were enrolled between December 2017 and September 2019. Follow-up lasted 90 days. Patients were randomized to receive 1 gr of intravenous clarithromycin or placebo once daily for 4 consecutive days.

In this sub-study, we evaluated the molecular responses to PF-06651600, an oral inhibitor of JAK3 and the tyrosine kinase expressed in hepatocellular carcinoma (TEC) kinase family, and PF-06700841, an oral TYK2/JAK1 inhibitor, versus placebo in nonlesional and lesional scalp biopsies of biopsy samples from patients with AA.

Gene-level analysis was performed. the results of exon-level analysis will be published separately (only preliminary results available so far).

Project description:In a randomized controlled trial, 82 older adults (>65y) with (or at risk of) undernutrition (n=82) were randomly allocated to 12 weeks of supplementation with a novel supplement (586 kcal, 22 g protein of which 50% whey and 50% casein, 206 mg ursolic acid, 7 g free BCAAs, 11 µg vitamin D) or standard care (600 kcal, 24g protein of which 100% casein, 4 µg vitamin D). Body weight increased significantly in the 12 weeks, both in the intervention group (+1.6 ± 0.2 kg, p<.0001) and in the standard care group (+1.8 ± 0.2 kg, p<.0001). Gait speed during 4m and 400m tests improved over time in the intervention group, whereas the standard care showed no improvements (time*treatment effects 400m: p=0.038 and 4m: p=0.048). Gene sets related to mitochondrial functioning were strongly upregulated in the participants receiving the intervention product. We showed that a novel oral nutritional supplement improves gait speed in older adults via improvements in mitochondrial functioning.

Project description:Bacterial-derived immune training agents are a showing promise in clinical studies for prevention of lower respiatory infections in high-risk infants but underlying mechanisms of this protective effect are unclear. To address this, we employed a systems-level analysis of transcriptional responses to immune stimuli (LPS and PolyI:C) in infants who participated in a clinical trial of one such agent called OM85 which was clinically effective. We identified key protected innate immune changes using a series of analytical approaches such as reduced inflammatory responses to LPS and enhanced antibacterial interferon responses potentially mediated through IRF7. We posit that immune training agents exemplified by OM85 potentially protect against infant severe lower respiratory infection principally via effects on innate immune responses targeting the bacterial components of the mixed respiratory viral/bacterial infections which are characteristic of this age group.
