Project description:The molecular mechanisms that control innate cell recruitment during chronic infection and inflammation, such as tuberculosis (TB), are incompletely understood. During TB, myeloid cells infiltrate the lung and sustain local inflammation. We identified microRNA (miR)-223 as one of the most abundant noncoding RNAs in lung parenchyma of TB patients and susceptible mice. MiR-223 controlled lung recruitment of myeloid cells, and consequently, neutrophil-driven lethal inflammation, by directly targeting the chemoattractants CXCL2, CCL3 and IL-6. Our study reveals an essential role for a single miR in TB. Moreover, we identify new targets for and assign novel biological functions to miR-223. By regulating leukocyte chemotaxis via chemoattractants, miR223 is critical for control of TB and probably other nonresolving inflammatory diseases. MicroRNAs expression analysis in whole blood from 8 TB and 8 TST+ subjects. The samples were collected at Makerere University in Kampala, Uganda. Gene expression analysis of whole blood and lung tissue of C57/BL6 and miR-223-/- mouse strains (8-12 weeks of age) after Mtb aerosol infection. Samples were collected at 7, 14 and 21 days post infection.

Project description:Whole blood transcriptional profiles of patients with (1) active pulmonary ['AdjuVIT active TB' and 'New active pulmonary'] and (2) extrapulmonary TB ['New active-extrapulmonary'] at time of diagnosis, (3) long-term recovery after treatment for active pulmonary TB ['AdjuVIT active TB'], (4) febrile illnesses presenting to hospital ['Fever mixed infection'] and (5) febrile pneumonia ['Fever pneumonia'] before antibiotic treatment, and (6) healthy vounteers. Each array sample represents a separate individual in each group. This submission includes two human whole genome Agilent Array Designs: A-MEXP-2104 and A-AGIL-28. Each of the individual raw array files are included as well as a single processed file representing the data matrix of all the merged and normalised data for the probes that are shared by the two array designs.

Project description:Tuberculosis-associated Immune Reconstitution Inflammatory Syndrome (TB-IRIS) is a common complication in HIV-TB co-infected patients receiving combined antiretroviral therapy (cART). While monocytes/macrophages play major roles in both HIV- and TB-infection individually, a putative contribution of monocytes to the development of TB-IRIS remains unexamined. We performed a genome-wide array analysis on MOs purified from peripheral blood mononuclear cells (PBMCs) obtained before initiation of combined antiretroviral therapy (cART) to verify whether the transcriptome of MOs was already significantly modulated (even before receiving cART) in HIV+/TB+ patients who later developed TB-IRIS compared to control HIV+/TB+ patients who did not develop the complication . The subjects under study included a subset of 18 TB-IRIS patients and controls matched for age, gender and CD4 count.

Project description:When exposed to M. tuberculosis, 25-50% become infected, of which 10-20% having several symptoms. Currently, there is no marker for judging the efficacy of M tuberculosis (Mtb) treatment, and it is judged based on the appearance of subjective symptoms and radiological findings. In this study, we attempted to establish a marker to distinguish patients who relapsed after TB treatment from patients who had recurrence after TB treatment using the information of circulating miRNA expression. Thirty two patients with tuberculosis infection aged 18 years or older were enrolled. 16 patients had onset or recurrence of TB and 16 patients with latent tuberculosis infection (LTBI) with positive test for Interferon gamma release assays (IGRA) and no chest X-ray examination. Total RNA from serum in exosome-rich fraction was extracted and then miRNA expression analysis was performed using a next-generation sequencer and NGS data analyzed using miRDeep2. Using expression information of nine miRNAs, TB and LTBI could be separated with 71.8% accuracy. The odds ratio also was 6.16, and the p-value was 3.20e-02, respectively. The expression of hsa-miR-122-5p was significantly different between TB and LTBI (p <0.05). Here we established a novel method to efficiently differentiate LTBI from patients with tuberculosis infection. Since this method can be performed in a low invasive manner such as blood sampling, it is promising tool for utilizing follow-up of TB treatment.

Project description:The differential expression of the long non-coding RNAs (lncRNAs) in the sputa from patients with Tuberculosis (TB) as (TB positive), community acquired pneumonia (CAP) and healthy individuals (CG) using lncRNA microarray scanning were evaluated. It was found that there were 2116 specific lncRNAs differentially expressed in the TB positive samples (1102 up-regulated and 1014 down-regulated), which accounted for 6.96% of total lncRNAs.

Project description:Abstract Background There is an urgent need for new, accurate, rapid, and affordable tuberculosis (TB) diagnostic tests. The aim of the present study was to use mass spectrometry to identify new preliminary candidate TB diagnostic protein biomarkers in saliva obtained from individuals with TB, and patients with other respiratory diseases (ORD). Methods Saliva samples were collected from 22 individuals who self-presented with symptoms suggestive of TB as part of a larger TB biomarker project. Purified salivary proteins were subjected to tryptic digestion peptides were analysed using a QExactive Orbitrap Mass Spectrometer. Identified proteins were subjected to gene ontology and ingenuity pathway analysis for functional enrichment analysis. Results 26 of the 652 identified proteins significantly discriminated individuals with TB from those with ORD after Benjamini Hochberg correction (5% FDR), with five of these proteins diagnosing TB with an AUC ≥ 0.80. A 5-protein biosignature comprising of P01011, Q8NCW5, P28072, A0A2Q2TTZ9, and Q99574 diagnosed TB with an AUC of 1.00 (95% CI, 1.00-1.00), sensitivity of 100% (95% CI, 76.2-100%) and specificity of 90.9% (95% CI, 58.7-99.8%) after leave-one-out cross validation. Conclusions We identified novel candidate salivary protein biomarkers and biosignatures with strong potential as TB diagnostic candidates. Our results are preliminary and require validation in larger studies.

Project description:Background Accurate assessment of treatment efficacy would facilitate clinical trials of new anti-tuberculosis drugs. TB patients exhibit altered peripheral immunity which reverts during successful treatment. We hypothesised that these changes could be observed in whole blood transcriptome profiles. Methods Ex vivo blood samples from 27 pulmonary TB patients were assayed at diagnosis and during conventional treatment. RNA was processed and hybridised to Affymetrix GeneChips, to determine expression of over 47,000 transcripts. Findings There were significant changes in expression of over 4,000 genes during treatment. Rapid, large scale changes were detected, with down-regulated expression of ~1,000 genes within the first week, including inflammatory markers such as the complement components C1q and C2. This was followed by slower changes in expression of different networks of genes, including a later increase in expression of B cell markers, transcription factors and signalling molecules. Interpretation The expression of many genes is drastically altered during TB disease, with components of the humoral immune response being markedly affected. The treatment-induced restoration reflects the simultaneous suppression and activation of different immune responses in TB. The rapid initial down-regulation of expression of inflammatory mediators coincides with rapid killing of actively dividing bacilli, whereas slower delayed changes occur as drugs act on dormant bacilli and as lung pathology resolves. Measurement of biosignatures during clinical trials of new drugs could be useful predictors of rapid bactericidal or sterilizing drug activity. Ex vivo blood samples analysed during TB treatment. These samples are from 9 successfully cured patients at diagnosis and end-of-treatment at 26 weeks.

Project description:Tuberculosis (TB) is one of the top ten causes of death worldwide and the leading cause of death from a single infectious agent. Globally, an estimated of 10 million people developed TB in 2018 according to WHO report. An estimated one third of all TB cases are not diagnosed or notified, partly due to the major limitations of current diagnostic tools. To achieve the goals of the WHO’s End TB Strategy, which targets for 2030 a 90% reduction in the number of TB deaths and an 80% reduction in the TB incidence rate compared with levels in 2015, diagnostic tools are critically important. Among the three diagnostic priorities identified by the WHO and the TB community is the development of a point-of-care biomarker-based non-sputum-based test to diagnose pulmonary TB, and ideally also extrapulmonary TB. To be successfully implemented at point-of-cares, a new test should use an easily accessible sample, such as urine, blood or breath condensate. Here, we explored whether bacilli-derived molecules released in the extracellular milieu during infection could be detected in the exhaled breath condensate, allowing a specific diagnosis of TB.
