Project description:To determine and dissect the extent of tumor, immunological, and stromal heterogeneity in CRC patients, we performed droplet-based scRNA-seq on 16 racially diverse, treatment naïve CRC patient tissue samples and seven adjacent normal colonic tissue samples (totaling 23 samples), yielded 49,589 single cells enabling high-resolution depiction of the cellular diversity and heterogeneity within the tumor and microenvironmental cells.

Project description:From a clinical and molecular perspective, colon cancer (CC) is a heterogeneous disease but to date no classification based on high-density transcriptome data has been established. The aim of this study was to build up a robust molecular classification of mRNA expression profiles (Affymetrix U133Plus2) of a large series of 443 CC and 19 non-tumoral colorectal mucosas, and to validate it on an independent serie of 123 CC and 906 public dataset. We identified and validated six molecular subtypes in this large cohort as a combination of multiple molecular processes that complement current disease stratification based on clinicopathological variables and molecular markers. The biological relevance of these subtypes was consolidated by significant differences in survival. These insights open new perspectives for improving prognostic models and targeted therapies.

Classification of serrated colorectal tumors

Project description:Serrated adenocarcinomas are morphologically different from conventional adenocarcinomas. The serrated pathway has recently been proposed to represent a novel mechanism of colorectal cancer (CRC) formation. However, whether they are biologically different and truly form a distinct subclass of CRC, is not known. This study shows that the gene expression profile of serrated and conventional CRCs differs from each others and that serrated CRCs are not only morphologically novel, but also biologically distinct subclass of CRC. Colorectal cancer classification based on gene expression is not associated with FOLFIRI response

Project description:Sadanandam et al. (2013) recently published a study based on the use of microarray data to classify colorectal cancer (CRC) samples. The classification claimed to have strong clinical implications, as reflected in the paper title: “A colorectal cancer classification system that associates cellular phenotype and responses to therapy”. They defined five subtypes: (i) inflammatory; (ii) goblet-like; (iii) enterocyte; (iv) transit-amplifying; and (v) stem-like. Based on drug sensitivity data from 21 patients, they also reported that the so-called stem-like subtype show differential sensitivity to FOLFIRI. This is the key result in their publication, since it implies a direct relation between the subtype and the choice of CRC therapy (i.e. FOLFIRI response). However, our analyses using the same drug sensitivity data and results from additional patients showed that the CRC classification reported by Sadanandam et al. is not predictive of FOLFIRI response. We used the classification algorithm to obtain CRC subtypes from our samples. Then we tested the subtype-drug response association performing a retrospective study.

Spatial characterization and stratification of colorectal adenomas by Deep Visual Proteomics

Project description:Background and Aims: Colorectal adenomas (CRAs) are precursor lesions that can progress to adenocarcinomas. Current clinical guidelines categorize patients into risk groups based on adenoma characteristics observed during index colonoscopy, but this may lead to overtreatment. Our aim was to establish a molecular feature-based risk allocation framework towards improved patient stratification. Methods: Deep Visual Proteomics (DVP) is a novel approach that combines image-based artificial intelligence with automated microdissection and ultra-high sensitive mass spectrometry. Here we used DVP on formalin-fixed, paraffin-embedded (FFPE) CRA tissues from nine patients. Immunohistological staining for Caudal-type homeobox 2 (CDX2), a gene implicated in colorectal cancer, enabled the characterization of cellular heterogeneity within distinct tissue regions and across patients. Results: DVP seamlessly integrated with current pathology workflows and equipment, identifying deleted in malignant brain tumors 1 (DMBT1), myristoylated alanine rich protein kinase C (MARCKS), and cluster of differentiation 99 (CD99) correlated with disease recurrence history, making them potential markers of risk stratification. The spatial and cell type specific capabilities of DVP uncovered a metabolic switch towards anaerobic glycolysis in areas of high dysplasia, which was specific for the cells with high CDX2 expression. Conclusion: The application of spatially resolved proteomics to CRA revealed three new potential markers for early-stage tumor development, and provided novel insights into metabolic reprogramming. Our findings underscore the potential of this technology to refine early-stage detection and contribute to personalized patient management strategies.
