Project description:BackgroundVenous thromboembolism (VTE) affects approximately 1-2 individuals per 1000 annually and is associated with an increased risk for pulmonary hypertension, postthrombotic syndrome, and recurrent VTE.ObjectiveTo determine risk factors, incidence, treatments, and outcomes of VTE through a 2-year surveillance program initiated in Durham County, North Carolina (population approximately 280,000 at time of study).Patients/methodsWe performed a retrospective analysis of data actively collected from three hospitals in Durham County during the surveillance period.ResultsA total of 987 patients were diagnosed with VTE, for an annual rate of 1.76 per 1000 individuals. Hospital-associated VTE occurred in 167 hospitalized patients (16.9%) and 271 outpatients who were hospitalized within 90 days of diagnosis (27.5%). Annual incidence was 1.98 per 1000 Black individuals compared to 1.25 per 1000 White individuals (p < 0.0001), and Black individuals with VTE were younger than White individuals (p < 0.0001). Common risk factors included active cancer, prolonged immobility, and obesity, and approximately half were still taking anticoagulant therapy 1 year later. A total of 224 patients died by 1 year (28.5% of patients for whom outcomes could be confirmed), and Black patients were more likely to have recurrent VTE than White patients during the first 6 months following initial presentation (9.4% vs. 4.1%, p = 0.01).ConclusionsOngoing surveillance provides an effective strategy to identify patients with VTE and monitor treatment and outcomes. We demonstrated that hospital-associated VTE continues to be a major contributor to the burden of VTE and confirmed the higher incidence of VTE in Black compared to White individuals.

Project description:Hydrologic changes modify microbial community structure and ecosystem functions, especially in wetland systems. Here, we present 24 metagenomes from a coastal freshwater wetland experiment in which we manipulated hydrologic conditions and plant presence. These wetland soil metagenomes will deepen our understanding of how hydrology and vegetation influence microbial functional diversity.

Project description:ObjectiveMultifaceted approaches to youth-violence prevention package evidence-based programs into initiatives that yield large-scale impact. This study assessed the impact of a package of evidence-based violence prevention programs, implemented as part of the North Carolina Youth Violence Prevention Center, on county-level violence indicators.MethodUsing growth-curve modeling, the target county was compared to all other counties in North Carolina and a comparison county.ResultsResults reveal downward trends on several county-level indicators (i.e., undisciplined/delinquent complaints, total delinquent complaints, juvenile arrests-aggravated assaults, and short-term suspensions) throughout the intervention period. However, statistical tests were unable to confirm that intervention-period scores on youth-violence indicators were significantly different than expected scores given the relationship between pretest and intervention-period scores in other North Carolina counties.ConclusionsAlthough additional administrative data points are needed to support the hypotheses, this study provides preliminary evidence of the effectiveness of North Carolina Youth Violence Prevention Center interventions.

Project description:We sought to examine how the impact of revocable behavioral interventions, e.g., shelter-in-place, varies throughout an epidemic, as well as the role that the proportion of susceptible individuals had on an intervention's impact. We estimated the theoretical impacts of start day, length, and intensity of interventions on disease transmission and illustrated them on COVID-19 dynamics in Wake County, North Carolina, to inform how interventions can be most effective. We used a Susceptible, Exposed, Infectious, and Recovered (SEIR) model to estimate epidemic curves with modifications to the disease transmission parameter (β). We designed modifications to simulate events likely to increase transmission (e.g., long weekends, holiday seasons) or behavioral interventions likely to decrease it (e.g., shelter-in-place, masking). We compared the resultant curves' shape, timing, and cumulative case count to baseline and across other modified curves. Interventions led to changes in COVID-19 dynamics, including moving the peak's location, height, and width. The proportion susceptible, at the start day, strongly influenced their impact. Early interventions shifted the curve, while interventions near the peak modified shape and case count. For some scenarios, in which the transmission parameter was decreased, the final cumulative count increased over baseline. We showed that the timing of revocable interventions has a strong impact on their effect. The same intervention applied at different time points, corresponding to different proportions of susceptibility, resulted in qualitatively differential effects. Accurate estimation of the proportion susceptible is critical for understanding an intervention's impact. The findings presented here provide evidence of the importance of estimating the proportion of the population that is susceptible when predicting the impact of behavioral infection control interventions. Greater emphasis should be placed on the estimation of this epidemic component in intervention design and decision-making. Our results are generic and are applicable to other infectious disease epidemics, as well as to future waves of the current COVID-19 epidemic. Developed into a publicly available tool that allows users to modify the parameters to estimate impacts of different interventions, these models could aid in evaluating behavioral intervention options prior to their use and in predicting case increases from specific events.

Project description:Much of the United States Atlantic coastline continues to undergo subsidence due to post glacial settlement and ground water depletion. Combined with eustatic sea level rise (SLR), this contributes to an increased rate of relative SLR. In this work, we utilize the ADvanced CIRCulation model to project storm surges across coastal North Carolina. Recent hurricanes Irene and Matthew are simulated considering SLR and subsidence estimates for 2100. Relative to present day conditions, storm surge susceptible regions increase by 27% (Irene) to 40% (Matthew) due to subsidence. Combined with SLR (+ 74 cm), results suggest more than a doubling of areal flood extent for Irene and more than a three-fold increase for Hurricane Matthew. Considering current regional population distributions, this translates to an increase in at-risk populations of 18% to 61% due to subsidence. Even further, exposed populations are projected to swell relative to Matthew and Irene baseline simulations (8200 and 28,500) by more than 70,000 in all SLR scenarios (79,400 to 133,600). While increases in surge inundation are driven primarily by SLR in the region, there remains a substantial contribution due to vertical land movement. This outlines the importance of exploring spatially variable land movement in surge prediction, independent of SLR.

Project description:Seafood mislabeling occurs in a wide range of seafood products worldwide, resulting in public distrust, economic fraud, and health risks for consumers. We quantified the extent of shrimp mislabeling in coastal and inland North Carolina. We used standard DNA barcoding procedures to determine the species identity of 106 shrimp sold as "local" by 60 vendors across North Carolina. Thirty-four percent of the purchased shrimp was mislabeled, and surprisingly the percentage did not differ significantly between coastal and inland counties. One third of product incorrectly marketed as "local" was in fact whiteleg shrimp: an imported and globally farmed species native to the eastern Pacific, not found in North Carolina waters. In addition to the negative ecosystem consequences of shrimp farming (e.g., the loss of mangrove forests and the coastal buffering they provide), North Carolina fishers-as with local fishers elsewhere-are negatively impacted when vendors label farmed, frozen, and imported shrimp as local, fresh, and wild-caught.

Project description:DEAD-box ATPases belong to an abundant class of proteins that are involved in virtually all aspects of RNA metabolism and are found in all kingdoms of life. When bound to a DEAD-box ATPase, the RNA substrate is forced into a kinked conformation that is incompatible with helical structures. Distortion of the RNA can result in unwinding of short RNA duplexes (helicase activity) or destabilize RNA-protein interactions, allowing DEAD-box ATPases to remodel mRNPs (RNPase activity). The RNPase activity makes DEAD-box ATPases suitable molecular building blocks for the implementation of checkpoints that confer directionality to the process of RNA biogenesis. Here, we provide data that characterizes the DEAD-box ATPase Dbp2 (SPBP8B7.16c) of the fission yeast Schizosaccharomyces pombe. Using ChIP-seq, we determined the sites where HTP-tagged Dbp2 associates with chromatin. ChIP-seq of Srp2-HTP is included as a reference protein that is known to associate with transcribing RNA polymerase II (RNAPII).

Project description:In North Carolina, USA, the SARS-CoV-2 Omicron variant was associated with changing symptomology in daily surveys, including increasing rates of self-reported cough and sore throat and decreased rates of loss of taste and smell. Compared with the pre-Delta period, Delta and Omicron (pre-BA.4/BA.5) variant periods were associated with shorter symptom duration.
