Needs, Gaps and Opportunities for Infectious Disease Research in British Columbia: A Perspective from Population and Public Health La recherche sur les maladies infectieuses en Colombie-Britannique: Besoins, lacunes et occasions dans l’optique de la sante publique et de la sante des populations

ABSTRACT:

Background

A review of infectious disease research activity and capacity was performed in British Columbia and linked to a process for identifying needs, gaps and opportunities from a public health perspective.

Methods

The study was organized in three phases: an environmental scan to describe current research activity in BC; a consultation to identify needs, gaps and opportunities with those conducting research (key informants) and the end users of research results (stakeholders); and a prioritization of the research needs emerging from the consultation.

Results

Analysis and synthesis of the consultation data resulted in the identification of nine research themes, which were prioritized in the following order: efficacy and cost-benefit, disease patterns, emerging infectious disease, immunology and vaccines, disease-specific research, health promotion and communications, safe food and water, knowledge translation research and genomics. Six capacity-building themes were also identified: attraction and retention, education and training, collaboration and networks, funding, dissemination of findings, and public health input, surveillance, informatics and databases.

Interpretation

The findings were helpful in developing a multi-disciplinary, multi-level infectious disease research agenda linking researchers in universities, hospitals and public health institutions with practitioners and policy-makers in British Columbia’s public health system. The approach is both feasible and important to undertake at the national level.

Project description:Contexte Alors que l’attention était focalisée sur la prise en charge des patients atteints de COVID-19, les conséquences potentiellement négatives des confinements successifs chez les patients atteints de maladies chroniques ont été peu explorées. Objectifs L’objectif de notre étude est d’évaluer l’impact du premier confinement lié à la COVID-19 chez les patients atteints de maladies chroniques en termes d’adhésion médicamenteuse, accès aux soins, règles hygiénodiététiques et santé mentale. Patients et méthodes Une étude de cohorte prospective téléphonique a été conduite entre le 14 avril 2020 et le 2 juin 2020 auprès de patients ambulatoires tirés au sort aléatoirement dans l’une des 8 cohortes (ou registres) régionales suivantes : artérite à cellules géantes, insuffisance cardiaque, dégénérescence maculaire liée à l’âge, fibrose pulmonaire idiopathique/hypertension artérielle pulmonaire, hémopathie maligne, hémophilie, sclérose en plaques, syndrome coronarien chronique. La non-adhésion médicamenteuse était définie comme une modification délibérée de la prise d’un ou plusieurs médicaments par le patient en dehors de tout avis médical, pharmaceutique ou infirmier. La détresse psychologique d’un patient était définie par un score ≥ 5 sur l’échelle Kessler Psychological Distress Scale K61. Un consentement oral était obtenu pour chaque participant à l’étude COVID-19 Lockdown Effects On Chronic Diseases (CLEO-CD ; NCT04390126). Résultats La population finale de l’étude comprenait 1274 patients d’âge moyen 66 ± 17 ans dont 55 % d’hommes. L’adhésion médicamenteuse était de 98 %. Les principales classes/molécules concernées par une non-adhésion des patients étaient : agents anticancéreux/immunosuppresseurs, anti-inflammatoires non stéroïdiens, antiangiogéniques, aspirine antiagrégante, inhibiteurs de l’enzyme de conversion. Parmi les 738 patients ayant au moins un rendez-vous médical programmé pendant la période, 305 (41 %) ont déclaré que le rendez-vous avait été annulé pour une raison qui n’était pas de leur ressort. Les principales modifications hygiénodiététiques étaient une détérioration du sommeil (qualité et/ou quantité ; 71 %), une augmentation > 25 % du temps passé devant un écran (46 %), une diminution de plus de 25 % de l’activité physique (46 %). Une détresse psychologique était présente chez 19 % des patients. En analyse multivariée, un habitat urbain (OR 1,76 [vs rural, < 2000 habitants] ; IC95 % 1,3–2,3 ; p = 10−4) et une détresse psychologique (OR 1,52 [vs score K6 < 5] ; IC95 % 1,1–2,2 ; p = 0,03) étaient des facteurs indépendants associés à la présence d’au moins un facteur délétère pour la santé des patients. Discussion/Conclusion Nos résultats sont rassurants en termes d’adhésion médicamenteuse. Chez les patients atteints de maladie chronique, une attention particulière est requise pour leur suivi s’ils habitent en milieu urbain et/ou s’ils présentent une détresse psychologique.

Project description:BackgroundClinical, functional, and cost-effectiveness outcomes from early intervention services (EIS) for psychosis are significantly associated with the duration of untreated psychosis (DUP) for the patients they serve. However, most EIS patients continue to report long DUP, while a reduction of DUP may improve outcomes. An understanding of different components of DUP and the factors associated with them may assist in targeting interventions toward specific sources of DUP.ObjectivesTo examine the components of DUP and their respective determinants in order to inform strategies for reducing delay in treatment in the context of an EIS.MethodsHelp-seeking (DUP-H), Referral (DUP-R), and Administrative (DUP-A) components of DUP, pathways to care, and patient characteristics were assessed in first episode psychosis (N = 532) patients entering an EIS that focuses on systemic interventions to promote rapid access. Determinants of each component were identified in the present sample using multivariate analyses.ResultsDUP-H (mean 25.64 ± 59.00) was longer than DUP-R (mean = 14.95 ± 45.67) and DUP-A (mean 1.48 ± 2.55). Multivariate analyses showed that DUP-H is modestly influenced by patient characteristics (diagnosis and premorbid adjustment; R 2 = 0.12) and DUP-R by a combination of personal characteristics (age of onset and education) and systemic factors (first health services contact and final source of referral; R 2 = 0.21). Comorbid substance abuse and referral from hospital emergency services have a modest influence on DUP-A (R 2 = 0.08). Patients with health care contact prior to onset of psychosis had a shorter DUP-H and DUP-R than those whose first contact was after psychosis onset (F(1, 498) = 4.85, P < 0.03 and F(1, 492) = 3.34, P < 0.07).ConclusionsAlthough much of the variance in DUP is unexplained, especially for help-seeking component, the systemic portion of DUP may be partially determined by relatively malleable factors. Interventions directed at altering pathways to care and promote rapid access may be important targets for reducing DUP. Simplifying administrative procedures may further assist in reducing DUP.
