Transitioning Out of the Coronavirus Lockdown: A Framework for Evaluating Zone-Based Social Distancing.

ABSTRACT: In the face of elevated pandemic risk, canonical epidemiological models imply the need for extreme social distancing over a prolonged period. Alternatively, people could be organized into zones, with more interactions inside their zone than across zones. Zones can deliver significantly lower infection rates, with less social distancing, particularly if combined with simple quarantine rules and contact tracing. This paper provides a framework for understanding and evaluating the implications of zones, quarantines, and other complementary policies.

Evaluating the Effectiveness of Social Distancing Interventions to Delay or Flatten the Epidemic Curve of Coronavirus Disease.

Project description:By April 2, 2020, >1 million persons worldwide were infected with severe acute respiratory syndrome coronavirus 2. We used a mathematical model to investigate the effectiveness of social distancing interventions in a mid-sized city. Interventions reduced contacts of adults >60 years of age, adults 20-59 years of age, and children <19 years of age for 6 weeks. Our results suggest interventions started earlier in the epidemic delay the epidemic curve and interventions started later flatten the epidemic curve. We noted that, while social distancing interventions were in place, most new cases, hospitalizations, and deaths were averted, even with modest reductions in contact among adults. However, when interventions ended, the epidemic rebounded. Our models suggest that social distancing can provide crucial time to increase healthcare capacity but must occur in conjunction with testing and contact tracing of all suspected cases to mitigate virus transmission.

Wage inequality and poverty effects of lockdown and social distancing in Europe.

Project description:Social distancing and lockdown measures taken to contain the spread of COVID-19 may have distributional economic costs beyond the contraction of GDP. Here we evaluate the capacity of individuals to work under a lockdown based on a Lockdown Working Ability index which considers their teleworking capacity and whether their occupation is essential or closed. Our analysis reveals substantial and uneven potential wage losses across the distribution all around Europe and we consistently find that both poverty and wage inequality rise in all European countries. Under four different scenarios (2 months of lockdown and 2 months of lockdown plus 6 months of partial functioning of closed occupations at 80%, 70% and 60% of full capacity) we estimate for 29 European countries an average increase in the headcount poverty index that goes from 4.9 to 9.4 percentage points and a mean loss rate for poor workers between 10% and 16.2%. The average increase in the Gini coefficient ranges between 3.5% to 7.3% depending on the scenario considered. Decomposing overall wage inequality in Europe, we find that lockdown and social distance measures produce a double process of divergence: both inequality within and between countries increase.

Pandemic responsiveness: Evidence from social distancing and lockdown policy during COVID-19.

Project description:We study changes in social distancing and government policy in response to local outbreaks during the COVID-19 pandemic. Using aggregated county-level data from approximately 20 million smartphones in the United States, we show that social distancing behaviors have responded to local outbreaks: a 1% increase in new cases (deaths) is associated with a 3% (11%) increase in social distancing intensity. Responsiveness is reinforced by the presence of public measures restricting movements, but remains significant in their absence. Responsiveness is higher in high-income, more educated, or Democrat-leaning counties, and in counties with low health insurance coverage. By contrast, social capital and vulnerability to infection are strongly associated with more social distancing but not with more responsiveness. Our results point to the importance of politics, trust and reciprocity for compliance with social distancing, while material constraints are more critical for being responsive to new risks such as the emergence of variants.

Effects of Cocooning on Coronavirus Disease Rates after Relaxing Social Distancing.

Project description:As coronavirus disease spreads throughout the United States, policymakers are contemplating reinstatement and relaxation of shelter-in-place orders. By using a model capturing high-risk populations and transmission rates estimated from hospitalization data, we found that postponing relaxation will only delay future disease waves. Cocooning vulnerable populations can prevent overwhelming medical surges.

Economic considerations for social distancing and behavioral based policies during an epidemic.

Project description:Public policies intended to induce behavioral change, specifically incentives to reduce interpersonal contacts or to "social distance," increasingly play a prominent role in public disease response strategies as governments plan for and respond to major epidemics. I compare social distancing incentives and outcomes under decentralized, full control social planner, and constrained social planner, without health class specific control, decision making scenarios. Constrained social planner decision making, based on non-health class specific controls, can in some instances make society worse off than decentralized decision making (i.e. no intervention). The oft neglected behavior of recovered and immune individuals is important for welfare and health outcomes.

Coronavirus infections and deaths by poverty status: The effects of social distancing.

Project description:We study the spread of COVID-19 infections and deaths by county poverty level in the US. In the beginning of the pandemic, counties with either very low poverty levels or very high poverty levels reported the highest numbers of cases. A U-shaped relationship prevails for counties with high population density while among counties with low population density, only poorer counties report high incidence rates of COVID-19. Second, we discuss the pattern of infections spreading from higher to lower income counties. Third, we show that stay-at-home mandates caused significantly higher reductions in mobility in high income counties that experienced adverse weather shocks than counties that did not. These effects are not present in counties with high poverty rates. Using weather shocks in combination with stay-at-home mandates as an instrument for social distancing, we find that measures taken to promote social distancing helped curb infections in high income counties but not in low income counties. These results have important policy implications for containing the spread of infectious diseases in the future.

Social distancing and mental health among pregnant women during the coronavirus pandemic.

Project description:BackgroundThe effect of social distancing due to the COVID-19 pandemic on the mental health of pregnant women is of particular concern, given potential effects on physical health, family functioning, and child development.MethodsPregnant women were recruited for the "Implications of and Experiences Surrounding being Pregnant during the COVID-19 Pandemic" study at Woman's Hospital in Baton Rouge, Louisiana. Participants enrolled at any point during their pregnancy and surveys were delivered weekly until the participant indicated that she had delivered her baby; a postpartum survey followed four weeks after delivery. This analysis includes 1037 participants with baseline, 596 with follow-up, and 302 with postpartum surveys. Questions on social distancing behaviors were asked at baseline and grouped based on whether they involved social distancing from work, friends and family, or public places. Symptoms of anxiety, stress, depression, and pregnancy-related anxiety were measured. Each type of social distancing was examined as a predictor of mental health using linear model with control for confounders.ResultsThe study population was largely white (84.1%), married (81.8%), and educated (76.2% with a bachelor's or higher degree). Women who were younger, Black, unmarried, or had less education or income reported fewer social distancing behaviors. Mean anxiety score in the highest quartile of overall social distancing was 8.3 (SD 5.6), while in the lowest quartile it was 6.0 (SD 5.0) (p < 0.01), while perceived stress postpartum and pregnancy-related stress were not associated with social distancing. Associations were substantially diminished when controlled for baseline levels of anxiety symptoms.ConclusionsGreater social distancing was associated with more anxiety symptoms, but worse mental health, particularly anxiety, may also have contributed to greater social distancing behaviors.

Understanding public perception of coronavirus disease 2019 (COVID-19) social distancing on Twitter.

Project description:ObjectiveSocial distancing policies are key in curtailing severe acute respiratory coronavirus virus 2 (SARS-CoV-2) spread, but their effectiveness is heavily contingent on public understanding and collective adherence. We studied public perception of social distancing through organic, large-scale discussion on Twitter.DesignRetrospective cross-sectional study.MethodsBetween March 27 and April 10, 2020, we retrieved English-only tweets matching two trending social distancing hashtags, #socialdistancing and #stayathome. We analyzed the tweets using natural language processing and machine-learning models, and we conducted a sentiment analysis to identify emotions and polarity. We evaluated the subjectivity of tweets and estimated the frequency of discussion of social distancing rules. We then identified clusters of discussion using topic modeling and associated sentiments.ResultsWe studied a sample of 574,903 tweets. For both hashtags, polarity was positive (mean, 0.148; SD, 0.290); only 15% of tweets had negative polarity. Tweets were more likely to be objective (median, 0.40; IQR, 0-0.6) with ~30% of tweets labeled as completely objective (labeled as 0 in range from 0 to 1). Approximately half of tweets (50.4%) primarily expressed joy and one-fifth expressed fear and surprise. Each correlated well with topic clusters identified by frequency including leisure and community support (ie, joy), concerns about food insecurity and quarantine effects (ie, fear), and unpredictability of coronavirus disease 2019 (COVID-19) and its implications (ie, surprise).ConclusionsConsidering the positive sentiment, preponderance of objective tweets, and topics supporting coping mechanisms, we concluded that Twitter users generally supported social distancing in the early stages of their implementation.

Polarization and public health: Partisan differences in social distancing during the coronavirus pandemic.

Project description:We study partisan differences in Americans' response to the COVID-19 pandemic. Political leaders and media outlets on the right and left have sent divergent messages about the severity of the crisis, which could impact the extent to which Republicans and Democrats engage in social distancing and other efforts to reduce disease transmission. We develop a simple model of a pandemic response with heterogeneous agents that clarifies the causes and consequences of heterogeneous responses. We use location data from a large sample of smartphones to show that areas with more Republicans engaged in less social distancing, controlling for other factors including public policies, population density, and local COVID cases and deaths. We then present new survey evidence of significant gaps at the individual level between Republicans and Democrats in self-reported social distancing, beliefs about personal COVID risk, and beliefs about the future severity of the pandemic.

Impact of Social Distancing Measures on Coronavirus Disease Healthcare Demand, Central Texas, USA.

Project description:Social distancing orders have been enacted worldwide to slow the coronavirus disease (COVID-19) pandemic, reduce strain on healthcare systems, and prevent deaths. To estimate the impact of the timing and intensity of such measures, we built a mathematical model of COVID-19 transmission that incorporates age-stratified risks and contact patterns and projects numbers of hospitalizations, patients in intensive care units, ventilator needs, and deaths within US cities. Focusing on the Austin metropolitan area of Texas, we found that immediate and extensive social distancing measures were required to ensure that COVID-19 cases did not exceed local hospital capacity by early May 2020. School closures alone hardly changed the epidemic curve. A 2-week delay in implementation was projected to accelerate the timing of peak healthcare needs by 4 weeks and cause a bed shortage in intensive care units. This analysis informed the Stay Home-Work Safe order enacted by Austin on March 24, 2020.
