Lessons learnt from easing COVID-19 restrictions: an analysis of countries and regions in Asia Pacific and Europe.

ABSTRACT: The COVID-19 pandemic is an unprecedented global crisis. Many countries have implemented restrictions on population movement to slow the spread of severe acute respiratory syndrome coronavirus 2 and prevent health systems from becoming overwhelmed; some have instituted full or partial lockdowns. However, lockdowns and other extreme restrictions cannot be sustained for the long term in the hope that there will be an effective vaccine or treatment for COVID-19. Governments worldwide now face the common challenge of easing lockdowns and restrictions while balancing various health, social, and economic concerns. To facilitate cross-country learning, this Health Policy paper uses an adapted framework to examine the approaches taken by nine high-income countries and regions that have started to ease COVID-19 restrictions: five in the Asia Pacific region (ie, Hong Kong [Special Administrative Region], Japan, New Zealand, Singapore, and South Korea) and four in Europe (ie, Germany, Norway, Spain, and the UK). This comparative analysis presents important lessons to be learnt from the experiences of these countries and regions. Although the future of the virus is unknown at present, countries should continue to share their experiences, shield populations who are at risk, and suppress transmission to save lives.

Project description:Although COVID-19 lockdown policies have improved air quality in numerous countries, there is a lack of empirical evidence on the extent to which recovery has resulted in air pollution rebound, and the differences and similarities among regions' recovery modes during the period of easing COVID-19 restrictions. Here, we used daily air quality data and the recovery index constructed by a city-pair inflow index for 119 cities in China to quantify the impact of recovery on air pollution from March 2 to October 30, 2020. Findings show that recovery has significantly increased air pollution. When the recovery level increased by 10 %, the concentration of PM2.5, SO2, and NO2 respectively deteriorated by 1.10, 0.33, 1.25 μg/m3, and the average growth rates of three air pollutants were about 3 %-6 %. Moreover, we used the counterfactual framework and time series clustering with wavelet transform to cluster the rebound trajectory of air pollution for 17 provinces into five recovery modes. Results show that COVID-19 has further intensified regional differentiations in economic development ability and green recovery trend. Three northwestern provinces dependent on their resource endowments belong to energy-intensive recovery mode, which have experienced a sharp rebound of air pollution for two months, thereby making green recovery more challenging to achieve. Three regions with a diversified industrial structure are in industrial-restructuring recovery mode, which has effectively returned to a normal level through adjusting industrial structure and technological innovation. Owing to local policies and the outbreak of COVID-19 in other countries, six provinces in policy-oriented and international trade-oriented recovery modes have not fully recovered to the level without COVID-19 until October 2020. The result highlights the importance of diversifying industrial structure, technological innovation, policy flexibility and industrial upgrading for different recovery modes to achieve long-term green recovery in the future.

Project description:BackgroundAnesthesia leads to impairments in central and peripheral thermoregulatory responses. Inadvertent perioperative hypothermia is hence a common perioperative complication, and is associated with coagulopathy, increased surgical site infection, delayed drug metabolism, prolonged recovery, and shivering. However, surveys across the world have shown poor compliance to perioperative temperature management guidelines. Therefore, we evaluated the prevalent practices and attitudes to perioperative temperature management in the Asia-Pacific region, and determined the individual and institutional factors that lead to noncompliance.MethodsA 40-question anonymous online questionnaire was distributed to anesthesiologists and anesthesia trainees in six countries in the Asia-Pacific (Singapore, Malaysia, Philippines, Thailand, India and South Korea). Participants were polled about their current practices in patient warming and temperature measurement across the preoperative, intraoperative and postoperative periods. Questions were also asked regarding various individual and environmental barriers to compliance.ResultsIn total, 1154 valid survey responses were obtained and analyzed. 279 (24.2%) of respondents prewarm, 508 (44.0%) perform intraoperative active warming, and 486 (42.1%) perform postoperative active warming in the majority of patients. Additionally, 531 (46.0%) measure temperature preoperatively, 767 (67.5%) measure temperature intraoperatively during general anesthesia, and 953 (82.6%) measure temperature postoperatively in the majority of patients. The availability of active warming devices in the operating room (p < 0.001, OR 10.040), absence of financial restriction (p < 0.001, OR 2.817), presence of hospital training courses (p = 0.011, OR 1.428), and presence of a hospital SOP (p < 0.001, OR 1.926) were significantly associated with compliance to intraoperative active warming.ConclusionsCompliance to international perioperative temperature management guidelines in Asia-Pacific remains poor, especially in small hospitals. Barriers to compliance were limited temperature management equipment, lack of locally-relevant standard operating procedures and training. This may inform international guideline committees on the needs of developing countries, or spur local anesthesiology societies to publish their own national guidelines.
