Project description:PurposeThe current health crisis has drastically impacted patient management in many fields, including neuro-urology, leading to a mandatory reorganization. The aim of this work was to establish guidelines regarding the prioritization and optimal timing of each step of neurogenic lower urinary tract dysfunction management.MethodsA steering committee included urologists and physical medicine and rehabilitation practitioners. Based on a literature review and their own expertise, they established a comprehensive risk-situation list and built a risk scale, allowing multiple other experts to score each clinical situation. New recommendations were generated using a Delphi process approach.ResultsForty-nine experts participated in the rating group. Among the 206 initial items, 163 were selected and divided into four domains, diagnosis and assessment, treatment, follow-up, and complications, and two sub-domains, general (applicable for all neurological conditions) and condition-specific [varying according to the neurological condition (spinal cord injury, multiple sclerosis, brain injury, Parkinsonism, dysraphism, lower motor neuron lesions)]. The resulted guidelines are expert opinions established by a panel of French-speaking specialists, which could limit the scalability of this work.ConclusionsThe present multidisciplinary collaborative work generates recommendations which complement existing guidelines and help clinicians to reorganize their patients' list in the long term with a personalized medicine approach, in the context of health crisis or not.

Project description:Introduction Des manifestations cutanées variées survenant lors d’infection par le SARS-CoV-2 ont été rapportées depuis mars 2020. Nous rapportons celles recueillies dans le cadre de l’étude COVID-Skin de la Société française de dermatologie (SFD). Matériel et méthodes Une enquête nationale a été lancée le 30 mars 2020 par la SFD, demandant aux médecins de signaler les cas de manifestations cutanées chez les cas suspects ou avérés de COVID-19, en utilisant un questionnaire standardisé. Nous rapportons les résultats pour les manifestations cutanées à l’exclusion des manifestations acrales décrites séparément. Les patients ont été classés COVID confirmée (RT-PCR et/ou sérologie positive), ou non confirmée (sérologie négative). Les patients n’ayant pas eu de test ou une PCR négative non confirmée par une sérologie négative n’ont pas été inclus dans l’analyse. Puis, les manifestations cutanées pour les cas avec photographies disponibles ont été classées par les auteurs sur la base de celle proposée par Galvan Casas et al (EMP, urticaire, éruptions vésiculeuses, nécrose/livedo). Résultats Nous avons recueilli 467 cas au 11 juin 2020 dont 311 cas de manifestations acrales étudiés spécifiquement. Parmi les 156 patients restants, 61 avait un diagnostic de COVID ni confirmé ni infirmé, 51 confirmé (45 RT-PCR positive et 7 avec une sérologie positive) et 44 non confirmés (sérologie négative). L’âge médian dans le groupe COVID confirmé était de 44,5 ans (IQR 30,3–63,5) vs 37,5 ans (IQR 28,5–47) chez les non confirmés, avec 61 % d’hommes (31/51) vs 67 % (29/43) respectivement. Dans le groupe COVID 49 % (25/51) des patients ont été hospitalisés, aucun dans l’autre groupe. La médiane de réalisation de la sérologie était de 28 jours (IQR 26,5–31) dans le groupe confirmé vs 22,5 jours (IQR 8,5–33,3). Une photographie était disponible chez 56 patients dont 38 dans le groupe COVID. Les principales manifestations cutanées et leur répartition entre les groupes sont résumées dans le tableau. Il n’a pas été mis en évidence de différence significative de la fréquence des différents types de manifestations entre les groupes (p = 0,127). Discussion Comparativement à l’étude de Galván Casas et al. portant sur 304 patients COVID confirmés ou probables en excluant les cas de manifestations acrales, nous trouvons une plus grande diversité de présentation clinique. Ils ont pu classer leurs patients en 4 types de manifestations : EMP 58 %, urticaire 21 %, éruptions vésiculeuses 11 % et nécrose livedo 7 %. Nous trouvons une proportion plus faible d’EMP, et d’éruptions vésiculeuse et une proportion comparable pour les 2 autres types (tableau 1). Nous avons observé divers autres types de manifestations cutanées chez 18 % dont certaines déjà rapportées. Compte tenu de l’hétérogénéité des manifestations et des diagnostics différentiels possibles, le lien formel entre l’infection et les éruptions cutanées restent à confirmer.

Project description:Health professionals are well positioned to protect children from environmental exposures to toxicants. Doing so may mean mobilizing for precautionary action well before conclusive evidence of adverse health impacts are documented in the community. This paper comments on the context within which health professionals work, and identifies strengths, barriers and guiding principles for action. Roles and strategies are identified for both medically and community-focussed health professionals. The prevention — treatment continuum (global action—policy and regulatory reform—local collective action—health protection—medical treatment) is explored. While health professionals traditionally deal with the treatment (downstream) end of the spectrum in addressing environmental health problems, there are ample opportunities for expanding this role to the prevention (upstream) end. Applicable strategies are organized into six major roles: surveillance, hazard assessment, research, education, public awareness, and advocacy. Advocacy is one of the most important roles since it involves upstream activities close to the source of the problem. While downstream endeavours have a legitimate place in the practice of health professionals, expanding this role to include upstream strategies, such as health promotion and advocacy, is essential if efforts to protect children’s health are to succeed in the long term. Project description:Background: Spinal fusion for scoliosis generates moderate to severe pain intensity. There are currently no instruments available to measure adolescents' satisfaction regarding post-spinal fusion pain management. Aims: To develop and validate a scale on satisfaction of adolescents regarding pain management following spinal fusion for scoliosis. Methods: A methodological design was used to develop and validate the French-Canadian scale "Satisfaction des Adolescents de la gestion de la Douleur postopératoire - Scoliose idiopathique (SAD-S)". A modified Delphi method, with seven healthcare professionals and 10 adolescents, was used to establish content validity of the SAD-S. A pre-test of the scale was conducted with 10 adolescents post-spinal fusion. The final version of the scale was validated through a pilot study with 98 adolescents following their surgery. Results: The SAD-S scale includes a total of 13 items. Principal component analysis yielded a two-factor structure (2 subscales): 1) Pain management education and 2) Education regarding medication. These two factors explained 47,8% of the total variance for satisfaction. A Cronbach's alpha of 0,84 was obtained for internal consistency. Conclusion: Validation of the SAD-S scale showed that it has good psychometric properties with this population. Further validation is required with a larger sample to pursue its validation.

Project description:Background A review of infectious disease research activity and capacity was performed in British Columbia and linked to a process for identifying needs, gaps and opportunities from a public health perspective. Methods The study was organized in three phases: an environmental scan to describe current research activity in BC; a consultation to identify needs, gaps and opportunities with those conducting research (key informants) and the end users of research results (stakeholders); and a prioritization of the research needs emerging from the consultation. Results Analysis and synthesis of the consultation data resulted in the identification of nine research themes, which were prioritized in the following order: efficacy and cost-benefit, disease patterns, emerging infectious disease, immunology and vaccines, disease-specific research, health promotion and communications, safe food and water, knowledge translation research and genomics. Six capacity-building themes were also identified: attraction and retention, education and training, collaboration and networks, funding, dissemination of findings, and public health input, surveillance, informatics and databases. Interpretation The findings were helpful in developing a multi-disciplinary, multi-level infectious disease research agenda linking researchers in universities, hospitals and public health institutions with practitioners and policy-makers in British Columbia’s public health system. The approach is both feasible and important to undertake at the national level. Project description:ObjectivesRelatively little is known about how health-care utilization differs among individuals with psychological distress compared to those with major depressive disorder (MDD).MethodsOntario participants of the Canadian Community Health Survey Cycle 1.2 (2002) were linked to health administrative data to follow their health-care utilization patterns for up to 15 years. Based on their survey responses, we classified individuals hierarchically into Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, criteria MDD, psychological distress (Kessler-6: 8 to 24), or an unexposed group with neither condition. We compared the rates of outpatient and acute care mental and nonmental health-related visits across the 3 groups over time using Poisson regression.ResultsAmong the 430 individuals with MDD, 668 with psychological distress, and 9,089 in the unexposed group, individuals with MDD and psychological distress had higher rates of health-care utilization than the unexposed overall and across time. The rates of psychiatrist visits for the MDD group were significantly higher than the other groups initially but declined over the follow-up. Conversely, the rates of psychiatrist visits among the psychological distress group increased over time and converged with that of the MDD group by the end of follow-up (rate ratioMDD vs. psychological distress at 1 year: 4.20 [1.97 to 11.40]; at 15 years: 1.53 [0.54 to 4.08]). Acute care visits were similar between the MDD and psychological distress groups at all time points.ConclusionsIndividuals with psychological distress required mental health care rivalling that of individuals with MDD over time, suggesting that even a cross-sectional assessment of significant psychological distress is a serious clinical concern.

Project description:The evidence from a large number of studies indicates that exposures to current outdoor air pollution increase respiratory morbidity in children. Children with asthma, and those with asthma-like symptoms but without a diagnosis of asthma, are considered to be at highest risk of experiencing shortterm and/or longer-term adverse health effects. Many outdoor air pollutants readily penetrate indoors. Indoor air quality can deteriorate quickly when persistent and uncontrolled emissions occur and the ventilation/air exchange rate is reduced. It has been estimated that children spend 90% of their time indoors, including in school buildings, vehicles and public indoor environments. Environmental tobacco smoke is a well-recognized persistent indoor air contaminant with adverse health effects in children of all ages. Uncontrolled moisture in the indoor environment is increasingly recognized to significantly increase the risk of respiratory morbidity in children. The evidence that air pollutants singly and in combination with other environmental factors elicit adverse health responses in asthmatic and non-asthmatic children and adolescents, appears irrefutable. Project description:ObjectiveSuicide prevention certainly includes a better knowledge of suicide risk in primary care. A number of international publications have shown interest in assessing this risk, but mostly through specific consultant populations: young patients, old patients, anhedonic, depressive, etc. Our study analyses suicide risk prevalence in patients consulting in general medicine for any somatic or psychiatric reason, their pathology or their age.MethodThis cross-sectional study was conducted with adult patients (827 subjects included) who were consulting a French generalist doctor panel randomly selected. They filled a validated self-questionnaire (aRSD) assessing their suicide risk in the 15 preceding days and providing professional and personal data.ResultsThe totally operable 757 files (483 female; 274 male) show that close to a quarter of consultants (24.3%) presents with a positive suicide risk in the 15 days preceding their consultation and 6.3%, reveal a severe risk (aRSD ≥ 7) with ideas and impulses to commit the act. When the reason to consult is psychiatric, 64.6% of these consultants have aRSD positive. One time out of two, the risk is even severe.ConclusionsThis data shows how important the suicide risk prevalence is in general medicine. It confirms the main role played by primary care patricians in acting to prevent suicide risk. This data also shows the contribution represented by a self-questionnaire that would rapidly assess the suicide intent while screening, it.

Project description:BackgroundRecent research found that physicians who completed medical school training at top-ranked U.S. medical schools prescribed fewer opioids than those trained at lower ranked schools, suggesting that physician training may play a role in the opioid epidemic. We replicated this analysis to understand whether this finding holds for Ontario, Canada.MethodsWe used data on all opioid prescriptions written by Ontario physicians between 2013 and 2017 from the Narcotics Monitoring System. Using the Corporate Provider Database and ICES Physician Database, which contain medical school of training, we linked patients who filled opioid prescriptions with their respective prescribing physician. Available data on Canadian medical school rankings were obtained from Maclean's news magazine. We used regression analysis to assess the relationship between number of opioid prescriptions and medical school ranking.ResultsCompared to the United States, average annual number of opioid prescriptions per physician was lower in Ontario (236 vs. 78). Unlike the United States, we found little evidence that physicians trained at lower ranked medical schools prescribed more than their top-ranked school counterparts after controlling for specialty and location of practice. However, primary care physicians trained at non-English-speaking foreign schools prescribed the most opioids even after excluding opioid maintenance therapy-related prescriptions.ConclusionThe role of medical school training on opioid prescribing patterns among Ontario physicians differs from that in the United States likely due to greater homogeneity of curricula among Canadian schools. Ensuring physicians trained abroad receive additional pain management/addiction training may help address part of the opioid epidemic in Ontario.

Project description:L’asthme est une maladie inflammatoire qui se traduit par une bronchoconstriction qui peut être modulée par l’action de médiateurs lipidiques endogènes (Prostaglandines, Leucotriènes, Éicosanoïdes). Bien que le mode d’action moléculaire et les effets physiologiques des époxy-eicosanoïdes (EET) soient relativement bien caractérisés (Morin et al, 2007 AJRCMB), les stimuli responsables de leur production sont mal identifiés. Le but du projet est d’identifier ces stimulis. Notre hypothèse de travail est qu’il existe une relation entre les effets de la Bradykinine et la synthèse des EET endogènes dans la paroi des bronchioles humaines. Les objectifs spécifiques sont : 1) Établir un nouveau modèle d’hyperréactivité bronchique (HRB) par la mise en culture de bronchioles humaines pendant 48h en utilisant l’ IL-1? et l’IL-13 en combinaison ou non avec le TNF?; 2) Quantifier les effets relaxants de la Bradykinine et les comparer à ceux du 14,15-EET exogène sur des bronches humaines hyper-réactives en absence ou en présence de différents inhibiteurs. Des mesures de tension mécanique dans des bains à organes isolés ont été réalisées sur des bronches provenant de résections pulmonaires humaines, disséquées et mises en culture pendant 48 h. Un traitement à l’IL-13 et au TNF-?; induit une hyperréactivité bronchique qui est abolie par un traitement de 48 h avec 100 nM de 14,15-EET. De plus, 1 ?M de Bradykinine ou 1 ?M de 14,15-EET relaxe le tonus contractile induit par 30 nM d’U-46619, un analogue du thromboxane A2. Cet effet relaxant est réduit de 25% par 10 nM d’Iberiotoxine (un bloqueur des BKCa). L’augmentation de la production et de la biodisponibilité du 14,15-EET est donc une stratégie qui pourrait se révéler intéressante pour minimiser l’HRB. En perspective, il nous reste à tester l’effet de la Bradykinine en présence de MSPPOH, un inhibiteur de CYP450 époxygénases ce qui minimiserait la production endogène des EET, et permettrait de confirmer notre hypothèse de travail.
