Neo-adjuvant Therapy and the Effect on Synchronous Metastatic Growth

Project description:Study Hypothesis • As well as in animal models as in patients with colorectal cancer resection of the primary tumor resulted in increase in vascular density, metabolism and secondary tumor growth of the distant metastases. These data strongly suggest an inhibitory effect of the primary tumor on the outgrowth of its metastases. In this study we investigate whether pre-operative treatment with the anti-angiogenic agent bevacizumab and/or chemotherapy before resection of the primary colorectal tumor shifts the balance between angiogenic and anti-angiogenic factors in favor of the anti-angiogenic factors and results in reduced growth of the liver metastases. Eligibility * Histological proven colorectal cancer without signs of bowel obstruction or bleeding * Synchronous liver metastases * WHO performance status 0-1 Treatment * Arm A: immediate surgery of the primary colorectal tumor, no neoadjuvant therapy * Arm B: neoadjuvant treatment with bevacizumab during 7 weeks prior to surgery of the colorectal primary * Arm C: neoadjuvant treatment with CAPOX during 7 weeks prior to surgery of the colorectal primary * Arm D: neoadjuvant treatment with bevacizumab and CAPOX during 7 weeks prior to surgery of the colorectal primary Primary endpoint Difference in response of liver metastases to resection of the primary tumor between the experimental groups and the control group, as determined by histopathological scoring of vascular density, apoptotic and mitotic index and by measurement of the metabolic activity of liver metastases by FDG-PET and SUV measurements. Project description:We used spatially resolved transcriptomics to define the cellular diversity within a sonic hedgehog (SHH) patient-derived model of Medulloblastoma (MB) and identify how cells specific to a transcriptional state or spatial location are pivotal in responses to treatment with the CDK4/6 inhibitor, Palbociclib. Our analysis reveals that SHH tumour in the mouse brain contains multiple malignant transcriptional states, recapitulating the extent of intratumoural cellular diversity identified in primary human SHH MB. Our study offers new insight into the previously described features of CDK4/6 inhibition in MB, with Palbociclib treatment inducing neuronal differentiation across all remaining cell types comprising the bulk of the SHH patient-derived orthotropic xenograft model. This study is, to the best of our knowledge, the first spatially resolved gene expression atlas of SHH PDOX MB and acts as proof-of-principle for the use of ST-seq in identifying spatially-organised tumour heterogeneity of MB.

Project description:Previously, we have demonstrated that the recombinant adeno-associated virus carrying the kringle domain 1 of hepatocyte growth factor (rAAV-HGFK1) gene is a potential anti-angiogenic approach for the treatment of hepatocellular carcinoma (HCC) and exerts direct inhibition effects on tumor cells. Here, using genome-wide expression profiling technology, coupled with RT-PCR validation, we investigated the differentially expressed genes and proposed the mechanisms involved in HGFK1 anti-angiogenic and anti-tumor cell signaling networks. Ribosome profiling of CLL patient samples

Project description:The functional consequences of many of the genetic changes, or combinations of genetic changes, in chronic lymphocytic leukaemia (CLL) are not known, but it is essential to discover these in order to understand the biology of the disease and the causes of individual variation in response to novel and conventional treatments. In this proof-of-principle study the translational landscape of CLL was determined for the first time by ribosome profiling. Individual variation in response to stimuli from the tumour microenvironment has not been systematically explored previously, but is likely to influence response to treatment and survival.

RNF20-mediated transcriptional pausing and alternative splicing of pro-angiogenic genes orchestrate vessel growth [06ATAC_siRNF20_HUVEC]

Project description:Signal-responsive gene expression is essential for vascular growth and patterning, yet the mechanisms allowing the integration of signaling inputs and transcriptional activities are largely unknown. Here we show that RNF20, the major E3 ubiquitin ligase of histone H2B, plays a key role during sprouting angiogenesis by mediating Pol II promoter-proximal pausing at pro-angiogenic genes. Furthermore, it orchestrates large-scale mRNA processing events, which change the bioavailability and function of critical pro-angiogenic factors, such as VEGFA. Mechanistically, we show that RNF20 restricts ERG-dependent gene activation through the VEGF-ERK1/2 axis while enabling Notch1 to bind to chromatin, thereby modulating the VEGF-Notch signaling circuits. Since perturbations of RNF20 are associated with cardiovascular malformations in human patients, our work may provide novel therapeutic avenues for diseases caused by vascular dysfunction.
